Salivary Glands
The parotid, submandibular and sublingual glands usually demonstrate only mild metabolic activity. Not infrequently, however, they can be even intensely avid.
As long as FDG uptake in these glands is bilateral, diffuse and not accompanied by a soft tissue lesion, even intensely avid salivary glands are considered normal.
Focal, unilateral, or asymmetric uptake (or uptake associated with a soft tissue abnormality) should always at least raise a concern for pathology — either malignancy or infection.

Even though extremely intense parotid uptake is present, its symmetry and lack of an associated soft tissue abnormality of the co-registered CT image indicates that this represents normal physiologic uptake.

It is not uncommon for both the parotid glands and the submandibular glands to be intensely avid. As long as their intensity is symmetric and there is no accompanying soft tissue abnormality on the CT images, this is presumed to represent normal physiologic uptake.

FDG uptake in the sublingual glands is extremely difficult to distinguish from normal uptake in the mylohyoid muscles. Mylohyoid uptake is very linear, paralleling the body of the mandible. Fortunately, both entities are benign.
