Baastrup Disease (Interspinous Uptake of FDG)

Baastrup’s Disease (“kissing spine”) is the result of degenerative changes between adjacent spinous processes and inflammation of the interspinous soft tissues. Patients are typically asymptomatic.

It has a very characteristic PET/CT appearance, with single or multiple levels of linear uptake present in the interspinous regions (best appreciated on the sagittal fused images).

It should not be confused with metastatic disease to the spinous process (always confirm the interspinous location of this uptake on the sagittal view, not in the axial plane).
