Bladder Hernia
While the CT appearance of this finding should never be missed, its striking PET appearance has led more than one PET/CT reader astray.
Usually, the supero-lateral margin of the bladder will be seen partially entering the inguinal canal. Occasionally, a significant segment of the bladder may extend deep into the hernia defect.
Caveat: Never be duped by an unusual focus of intense metabolic activity anywhere in the body. Always rule out every possible non-neoplastic explanation before raising the specter of cancer.

Axial CT images confirm the hypermetabolic density to be located within the left inguinal canal (note the fat-containing right inguinal hernia).
Case 1 (Image 3)
Axial CT images just superior to Image 2 demonstrate extension of the left superior aspect of the urinary bladder toward the left inguinal region.
Case 1 (Image 4)
Coronal images confirm the connection of the hypermetabolic focus directly to the urinary bladder. This is the typical appearance of an inguinal hernia containing a segment of the urinary bladder.
