Fractures
Healing fractures can demonstrate moderate to intense focal FDG uptake, which usually resolves within 8-12 weeks. Fully healed fractures are non-avid.
To exclude a pathologic fracture, carefully evaluate the CT images for evidence of an underlying bone lesion (also try to elicit a history of recent trauma to the region).

Corresponding sagittal images of this FDG-avid L3 vertebral body fracture. In the absence of a history of recent trauma, this would be concerning for a pathologic fracture. In this case, the patient reported a significant fall 3-weeks previously, requiring an ER visit. This was a benign traumatic burst fracture.

Corresponding coronal images confirm intense linear uptake along the superior endplate of the vertebral body without a corresponding osseous abnormality on the bone widows. This linear configuration of metabolic activity along the endplate is most consistent with an impending/early compression fracture.

A follow up PET/CT scan demonstrates insufficiency fractures of the right and left sacrum (fracture lines visible), intensely FDG-avid, with areas of ill-defined sclerosis present. Given the patient’s clinical history, these are most consistent with post-radiation insufficiency fractures.
