Hypermetabolic Geographic Focal Fatty Infiltration of the Liver

On very rare occasion, focal fatty infiltration will present as an FDG-avid lesion on the PET portion of the exam.  

The finding of hypermetabolic geographic fatty infiltration, however, is so uncommon that we do not include it in our reports in the differential diagnosis for an FDG-avid liver lesion.

While biopsy will almost always be indicated, clues to this potential diagnosis include the lack of a well-defined lesion on the co-registered CT images and the “lesion” being located in a classic location for focal fatty infiltration.

If there is a real suspicion for this rare finding, MRI evaluation demonstrating a homogeneous drop in signal on out-of-phase imaging may confirm the diagnosis.
